Mandatory Paperwork and Training

Each year catechists and aides must submit at least the following three forms. These are to be provided by the Director of Religious Education (DRE).

Covenant Form
General Acknowlegment Form
Compensation Form (to receive a stipend) or Volunteer Form (to donate services)

Catechists and aides wishing to be paid a stipend must complete (only once):

Federal W-4
Iowa W-4
I-9 Employment Eligibility Verification Form (copies of ID with a picture and Social Security number must accompany this form. For example, just a copy of one's passport or copies of both a driver's license and Social Security card willl suffice.)

New catechists and aides must complete (only once):

Verification Form (must be copies back-to-back with the Background Check Form by the DRE when submitted to the Archdiocese)
Background Check Form (must be copied back-to-back with the Verification Form by the DRE when submitted to the Archdiocese)
